TO GOAL™: Asthma (IMPACT)
2008-2010
IMPACT™ utilized the successful quality improvement model developed in TO GOAL™ to provide asthma quality improvement guidance, support and resources to the 75 participating family physicians and their care teams. Elements of the Patient Centered Medical Home (PCMH) were incorporated throughout the program, providing teams with an introduction to the fundamentals of this model of care. Physicians who participated in IMPACT™ qualify for MOC Part IV credit.
Nurse team members played a crucial role in the success of this project. In addition to physician leaders, each team designated a lead nurse who was provided access to a nurse coach and certified asthma educator. Nurses also participated in continuing education through the National Respiratory Training Center and monthly conference calls with expert faculty.
