TO GOAL™: COPD

Effective management of Chronic Obstructive Pulmonary Disease (COPD) is dependent on physicians and their care teams’ ability to translate guidelines and evidence-based medicine into routine treatment practices. As seen in TO GOAL™ Cardiovascular Disease, physicians are most successful when utilizing a guided, collaborative approach that utilizes a comprehensive curriculum of disease specific knowledge, physician leadership and practice change training, along with successful implementation strategies to effect meaningful improvements in patient outcomes. This was further validated in Improving Asthma Care and Treatment (IMPACT), the asthma expansion of TO GOAL™. Further strengthening this model, nurse leaders received tailored education, faculty support and coaching that improved team dynamics and most importantly empowered the nurses to take on a more active role in patient care and education. This successful integrated team approach was used in TO GOAL: COPD.
